We participate with most major health insurance companies.  Please check your insurance benefits to be certain we are participating providers in your plan.  We require all co-pays and/or deductibles be paid at the time of service.  If your insurance plan requires an authorization from your primary care provider, please secure this prior to your appointment.  If you have questions concerning the financial policies of our office, please do not hesitate to contact our patient accounts representative.

We ask that you bring your insurance card(s) with you at the time of every appointment.
